Leicestershire SN Transitions Parent Carer Support Group

Leicestershire SN Transitions Group is open to all parents and carers of young people, aged 14 to 25 who are entering or going through the Transitions Stage (preparing for adulthood). The Group recognises the particular confusions and stresses that Transitioning from Children's to Adult Services across education, health, social care and state benefits can bring and aims to provide information and support to families as well as monitoring 'what is happening' at NHS and Local Authority level.

We cater for all disabilities and for young people in both mainstream and special schools. We meet monthly during term time at Birket.

We now meet monthly (during term time) at Birkett House Special School in Wigston and regularly have professionals come to talk to us from Education and Social Care.
