Care2Communties deliver innovate support services to maximise the wellbeing of people living in their own homes, to support (unpaid) carers and connect communities.
• Self-Care – We encourage and support people to care for themselves as much as possible, increasing independence
• Assistive Technology – We use assistive technology to ensure people can stay in their own home and remain as independent as possible, for as long as possible
• Paid Support – We offer paid support to meet the needs and outcomes of people.
As people’s needs increase, traditionally there would be a need for increased paid support, we continually go through our support cycle to ensure that people are encouraged with self-care, supporting independence and opportunities for people to maintain, relearn or regain skills
• Family, Friends & Connections – We support people to maintain friendships and relationships
